卵巢癌
卵巢癌是一种起源于女性卵巢的恶性肿瘤,根据组织来源不同,一般分为上皮性癌、生殖细胞恶性肿瘤、性索间质细胞肿瘤,其中上皮性癌是卵巢恶性肿瘤中最常见的类型,约占90%。由于卵巢位于盆腔深部,肿瘤早期无特异性的症状,患者在就诊时,多已为晚期,预后较差,严重威胁患者生命安全。
卵巢癌的治疗主要包括手术和化疗,手术目的是切除肿瘤、明确诊断、准确分期、判断预后和指导治疗。近年来,随着对卵巢癌基因分子层面认识的加深和靶向药物逐渐应用到临床,卵巢癌的精准治疗手段也越来越丰富。
PARP抑制剂是近年来在卵巢癌治疗中取得显著进展的一类药物,它们通过抑制PARP酶活性,对BRCA突变或同源重组修复缺陷(HRD)的卵巢癌细胞具有杀伤作用。目前,多种PARP抑制剂如奥拉帕利、尼拉帕利、帕米帕利等已获批用于卵巢癌的治疗,尤其在BRCA突变或HRD阳性的患者中显示出较好的疗效。此外,针对FRα的ADC药物索米妥昔单抗(mirvetuximab soravtansine,MIRV)在卵巢癌治疗中显示出一定的效果。
